Facilitate PATH & MAPS - The Arc's Statewide Facilitators' Network

When a person goes through a person centered planning process, it increased the chances that they will achieve a life goal, realize a dream or build relationships.  PATH (Planning Alternative Tomorrows with Hope) and MAPS are person centered planning tools developed by John O’Brien, Jack Pearpoint, Marsha Forest, and Judith Snow from the Centre on Inclusion in Canada.

The Arc has been facilitating PATH for a number of years now, and on April 15, 2007 was officially approved by Medicaid and the Developmental Disabilities Services Division (DDSD) to offer Personal Plan Facilitation.  Personal Plan Facilitation offers individuals receiving waiver services the opportunity to utilize a variety of person-centered planning techniques and processes to explore and identify their vision and goals for the future. 

The Arc has created a Facilitator’s Network that includes individuals from around the state who have been trained to facilitate the PATH/MAPs processes.  Training includes experiencing your own PATH or MAP and mentorship by other facilitators.  The network meets in Albuquerque on a quarterly basis.
